Apply for nursing home long-term care: Nursing Home Long-Term Care Admissions Application
We accept referrals for admission from a variety of sources:
- Hospitals
- Senior care facilities (i.e., assisted living)
- Physicians
- Family members
- Seniors in the community
Admitting Applicants Applicants are admitted based on their need for long-term care services, the availability of beds, and the assessed level of care. Russian-speaking seniors should contact our Bilingual Services Program for admission to this program.
Preparation for Admission To help HRC prepare both clinically and financially for a senior's care, our admission procedures include:
- An application to gather medical care history and financial data.
- An interview with a social worker to assess the appropriate level of care required, share our philosophy, and answer questions or concerns about life at HRC.
Assistance Throughout Application Process An assigned social worker follows the senior throughout the application process to assist the senior and family members in decision making and to provide support during the transition period from application to admission.
