Social Services

HRC's master's-level licensed social workers provide a wide range of services to patients/residents and their family members throughout their time at HRC. The Social Services staff aims to assist seniors and their families through the many transitions that aging can bring. Through assessing needs, providing information, and offering emotional support, they serve as advocates and primary liaisons between patients/residents, their families, and HRC. 

In long-term care, the admission social workers maintain close contact with seniors and their family members throughout the application and admission process. They provide support, referral information, and help in planning for the transition to HRC.

After admission to long-term care, the unit social worker assumes responsibility for working with newly admitted residents and their families by providing help with orientation and support throughout the adjustment process. Social workers continue to work with all residents and family members by providing emotional support and care management on an ongoing basis throughout each residents stay at HRC.

The RSU and MACU social workers support the patient's admission to either of the units.  They help complete an admission package, including a psychosocial assessment, to help determine a patients needs during their stay. They then work closely with the patient and family to identify resources and facilitate the discharge planning process.
