More than just a place to live, Center Communities of Brookline is designed to support seniors’ health and well-being. This includes the Sloane Family/Century Bank Primary and Specialty Care Center at 100 Centre Street.
Seniors who join our medical practice enjoy a primary care team that is focused on each individual’s values, goals, and desires for care. You and your doctor are partners in your care.
We offer:

- Routine care for chronic medical conditions
- Urgent care appointments
- A clinician on call 24/7
- On-site lab services
Our geriatricians and nurse practitioners provide coordinated care with the medical specialists who also deliver their services on-site, including:
- Occupational therapy

- Physical therapy
- Psychiatry
- Social work services
Dr. Jennifer Rhodes-Kropf is on the faculty at Harvard Medical School and board-certified in internal medicine, geriatrics, hospice, and palliative medicine/chronic pain management. She works closely with a team that includes a nurse practitioner, psychiatrist, clinical coordinator, social worker, medical assistant, physical therapist, occupational therapist, and home nurses. And they are located right on the premises. Compare that with driving to Longwood and then sitting in a crowded waiting room. Dr. Rhodes-Kropf is part of the BIDMC/HSL division of geriatrics. She is also credentialed with Partners Healthcare.
Should the need arise, our Hebrew SeniorLife geriatricians are able to provide continuous, coordinated, and seamless care, whether the resident is in a Boston hospital, at one of Hebrew SeniorLife’s post-acute care facilities, or at home.
