Coordinated Care in Your Community

At Simon C. Fireman Community, you have access to a full range of support and services that ensure continuity of health care, encourage prevention, and emphasize wellness. You’ll receive priority access to all Hebrew SeniorLife’s Harvard Medical School-affiliated services, including those offered at Hebrew Rehabilitation Center in Boston and Dedham.

On-site offerings include:

  • Our Vitalize 360 wellness program
  • Social services staff to help you coordinate care
  • A mobile podiatry service
  • Regular blood lab and blood pressure clinic

Simon C. Fireman Community residents also have access to a wellness nurse and wellness coordinator through our award-winning ‘Right Care, Right Place, Right Time’ – or R3 – care coordination initiative. Through this program, a wellness team within the community will work with you, your primary care provider, and other community organizations to keep you healthy. The goal is to keep you in your home and to reduce unnecessary costs.
