Health Allies embeds wellness teams in affordable senior housing to help older adults stay independent while reducing hospital use and health care costs.

Imagine a dedicated wellness team embedded right in a housing community — helping residents navigate the health care system, manage chronic conditions, and stay independent, with improved health outcomes and quality of life.

Health Allies (formerly Right Care, Right Place, Right Time) is an innovative program from Hebrew SeniorLife that brings healthy aging teams into affordable senior housing communities. We’ve spent almost a decade proving that integrating housing and health care improves lives and lowers health care costs — and we’re looking for new partners to bring this model to their communities and the people they serve.

The program has won the Pioneer Institute 2017 Better Government Competition, The John A. Hartford Foundation 2021 Business Innovation Award, and the 2025 Association of Jewish Aging Services Innovation Igniter Award.

How Health Allies Works

Our nurse care manager and wellness coordinator work on-site alongside the existing housing team, becoming trusted, familiar faces to residents. They build relationships — learning residents’ wishes, priorities, and goals, then developing individualized plans that support their health, independence, and engagement.

Residents receive:

  • Connections to services, support, and resources addressing their needs, such as mental health, brain health, nutrition, and chronic disease management
  • Coordinated care transitions after a hospital or rehab stay
  • A link between their primary care and other providers
  • On-site fitness classes, health education, and wellness programming

HSL has been an amazing partner. They’re in the same mission space and have outstanding quality outcomes—from reducing ED visits to improving health outcomes in the living spaces, to getting previously homebound residents engaged in community events in these buildings.

  • 16% decline in inpatient hospitalization rate
  • 19% decline in trips to the emergency room
  • 22% decline in 30-day hospital readmission rates
  • 25% decline in total days admitted to a hospital per resident
  • 90% of identified service needs met
  • 87% of residents would recommend the program to a friend
