Making a Referral to Our Chronic Care Hospital

We accept referrals for admission from a variety of sources:

  • Hospitals
  • Senior care facilities (i.e., assisted living)
  • Physicians
  • Family members
  • Seniors in the community

Admitting Patients

Patients are admitted based on their need for long-term chronic care, the availability of beds, and the assessed level of care. Russian-speaking seniors should contact our Russian Language Program for admission to this program.

Assistance Throughout Referral Process

An Admissions Coordinator follows the senior throughout the referral process to assist the senior and family members in decision making and to provide support during the transition period from referral to admission. Review Hebrew Rehabilitation Center Financial Assistance Programs.

Medically Acute Admission

HRC offers a medically acute care program for post-acute patients with complex medical conditions or multiple acute or chronic illnesses.

Evaluation prior to admission

A HRC clinical liaison will evaluate patients prior to admission. Admissions are based on clinical information. Once a patient is deemed clinically appropriate, insurance benefits and eligibility are reviewed.
