Hebrew SeniorLife and Schochet Partner to Bring Healthy Aging Program to Fresh Pond Apartments
Schochet’s Fresh Pond Apartments is the first multi-family property to offer Hebrew SeniorLife’s Right Care, Right Place, Right Time (R3) program.
Hebrew SeniorLife and Schochet this month introduced the nationally recognized Right Care, Right Place, Right Time (R3) program at Schochet’s Fresh Pond Apartments, a 504-unit affordable housing complex in Cambridge, MA. This marks the first time the R3 program has been made available at a multi-family property. The program offers the 230 older adults at Fresh Pond Apartments resources to live healthy, active, vibrant lives.
Working with the on-site Schochet resident services team at Fresh Pond, the R3 program will offer place-based services to improve older adults’ emotional and physical well-being, increase independence and longevity, and reduce health care costs. Hebrew SeniorLife will offer an on-site nurse, blood pressure clinics, health and wellness education, exercise classes, assistance with benefits, and access to resources.
Schochet President and CEO Richard Henken said, “Enabling senior residents to remain in their homes and communities is an extension of two of Schochet’s frequently stated corporate goals: enhancing quality of life and treating people with respect. Numerous studies have demonstrated that seniors who remain safely in their own home environment for as long as possible tend to experience a longer life expectancy, better health outcomes, and a delay in cognitive decline. We are proud of the strides we have been able to make towards these goals through our partnership with Hebrew SeniorLife at two of our senior affordable housing properties and especially proud to introduce these services at our first multi-housing property, extending the ability of our senior residents to remain a vibrant part of our Fresh Pond community.”
“Hebrew SeniorLife is dedicated to providing older adults with personalized care and services, allowing them to live independently and well through our pioneering R3 program,” said Stephanie Small, R3 executive director at Hebrew SeniorLife. “By partnering with Schochet, we can better equip older adults with the health and wellness resources that they need to live independent, quality lives.”
This is the third Schochet-managed property and first multi-family housing property to offer R3, an innovative program that integrates wellness teams into senior housing communities. Others include Riverside Towers, a joint venture with Jonathan Rose Companies in Medford, MA, and Retirement Housing Foundation’s Hamilton Wade/Douglas House in Brockton, MA.
R3 was created by Hebrew SeniorLife to support residents in proactively managing their health by effectively linking housing and health care. The goal of R3 is to improve lives, reduce costs, and help older adults stay independent and at home longer. The program won the Pioneer Institute 2017 Better Government Competition, the John A. Hartford Foundation 2021 Business Innovation Award, and the 2025 Association of Jewish Aging Services Innovation Igniter Award. The R3 model includes a nurse care manager, wellness coordinator, and fitness specialist working on-site with the existing housing team to provide supportive services to residents.
Staff reach out regularly to individual residents to engage them around their health and wellness. Through these conversations, they work to understand the barriers residents might face in remaining healthy, then follow up to address needs and connect them to resources. The wellness team also provides individualized case management and support, all centered around the resident’s choice and preferences. The Fresh Pond healthy aging team members will help residents aged 62 and older with accessing and coordinating these services.
In addition to the wellness teams’ work, all community staff — including maintenance, administrators, and social workers — are trained to contribute to care coordination efforts.
