Hebrew SeniorLife’s Right Care, Right Place, Right Time is now Health Allies

Hebrew SeniorLife’s Right Care, Right Place, Right Time (R3) has a new name — Health Allies.

Health Allies brings healthy aging teams into affordable senior housing communities to help older adults navigate the health care system, optimize their health and well-being, remain independent longer, and enjoy their best possible lives. It is currently in place at 18 affordable housing communities across Greater Boston.

“This new name better represents how our healthy aging teams partner with residents, introducing an integrated approach that promotes proactive health management, self-empowerment, and aging with optimal independence and dignity,” explained Health Allies Executive Director Stephanie Small.

Health Allies nurse care managers and wellness coordinators team up with older adults to help them connect to services, support, and resources; manage new or chronic conditions; and navigate care transitions after a rehabilitation or hospital stay — all with a focus on health, engagement, and independence. By addressing issues before they become health emergencies and supporting care transitions, Health Allies helps reduce costs across the health care system.

Health Allies also includes fitness classes, educational workshops, health clinics, and more to help residents manage their health and avoid loneliness and isolation.

Residents of communities with Health Allies tend to live at home longer, be better prepared to manage their own health, and live more fulfilled lives. Research shows:

• A 16% decline in inpatient hospitalization rate
• A 19% decline in trips to the emergency room
• 87% of participants would recommend the program to a friend.

The initiative previously won the Pioneer Institute 2017 Better Government Competition, The John A. Hartford Foundation 2021 Business Innovation Award, and the 2025 Association of Jewish Aging Services Innovation Igniter Award.
