The Deanna and Sidney Wolk Center for Memory Health at Hebrew SeniorLife provides outpatient memory care services, in person and virtually, for people living with cognitive symptoms or disorders at any stage — and for their families and caregivers.
We know caring for a person showing signs of dementia, due to Alzheimer’s disease or another cause, can be overwhelming. We are here to help you cope with the stress of a loved one’s condition and provide the resources you need to make informed decisions.

- Behavioral and Cognitive Neurology
Beyond dementia testing and diagnosis, our neurologist helps patients and families get a better picture through consultations to:
- Assess cognitive function
- Identify early signs of dementia
- Address behavioral changes
- Diagnose and treat medical conditions and co-morbidities
- Recommend personalized, effective interventions and lifestyles
- Cognitive Health and Dementia Care Management
Our care manager will help you develop a plan by identifying unmet needs and developing a personalized, structured care plan to address the medical, behavioral, and social elements of daily life.
- Family and Caregiver Support
Our programs include individual and family counseling as well as caregiver support groups – all of which are conveniently available virtually through online telehealth.
- Resource Guidance
Our resource specialist can help you identify local programs and services to promote wellbeing – whether you are coping with cognitive concerns or caring for someone who is.
These outpatient services may be used individually, or in combination. Services are covered by Medicare and most insurances, and most are available virtually.
